Administrative Overview
Offices are located in Bernalillo, Dona Ana, Sandoval, Sierra, Socorro
and Valencia Counties. Staff is on call 24/7.
Administrative Hours
M - F: 8 am - 5 pm. A nurse is on call 24/7 for medical assistance and
emergencies.

In-Home Hospice Care

| DESCRIPTION |
Provides medical services related to the terminal diagnosis
including medications, home equipment such as reclining beds,
walkers, and wheelchairs, and an array of professional visiting
staff and volunteers. Attempts to make the patient as pain-free
and comfortable as possible while providing family members and
significant others with the help they need. |
| ELIGIBILITY |
Must have physician's certification
that patient has a terminal illness with an estimated prognosis of less than 6 months, must
be receiving only palliative (comfort) care. Applicant must have
a primary caregiver. |

Medicare, Medicaid, private insurance and private pay. |

HOME HEALTH CARE

| DESCRIPTION |
Provides intake planners to design customized comprehensive home
health care: In-home nursing, in-home medical equipment,
assistance with daily living and hospice care. Caretaking is provided by an employee
of Ambercare. Alternatively, a family member can become
a paid caretaker. |

Must have a doctor's order for home health care. |

Fees
vary and are based on negotiated contracts. Accepts Medicare,
Medicaid, private insurance and
private pay. The Medicaid Personal Care Option (PCO) is
available to most Medicaid
recipients who receive SSI, are over 21 and need assistance with
at least 2 activities of daily living. D and E waiver. |
