| DESCRIPTION | Provides customized information packet to applicants who submit an information form about their medication needs. Informs the qualified applicant about programs that can assist them to fill their prescriptions. Simplifies the application procedure and provides general guidance in the application process. |
| ELIGIBILITY | No prescription outpatient drug coverage, income is at a level that causes a hardship when the patient is required to purchase the medication at retail prices and patient doesn't qualify for 3rd party programs that provide for prescription medication coverage. |
| FEES | $5 for each prescription inquiry, refunded upon request if application not approved. |
| HOURS | M-F: 8:00 AM - 5:00 PM |
| APPLICATION | Complete and submit a medication information form (available from the agency's web site at www.themedicineprogram.com) along with a $5 processing fee payable to The Medicine Program. A personal letter may be substituted for the application form when it includes the name, address and phone number of the person taking the medication and the name of his or her physician and the name of the medication. |
