| AGE RANGE | All ages | | DESCRIPTION | Provides medical services related to the terminal diagnosis including medications, home equipment such as reclining beds, walkers, and wheelchairs, and an array of professional visiting staff and volunteers. Attempts to make the patient as pain-free and comfortable as possible while providing family members and significant others with the help they need. | | ELIGIBILITY | Must have physician's certification that patient has a terminal illness with an estimated prognosis of less than 6 months, must be receiving only palliative (comfort) care. Applicant must have a primary caregiver. | | FEES | Fees vary and are based on negotiated contracts. The Medicaid Personal Care Option (PCO) is available to most Medicaid recipients who receive SSI, are over 21 and need assistance with at least 2 activities of daily living.
